Mechanism of Action
Dexamethasone is a potent glucocorticoid with minimal mineralocorticoid activity. It exerts its effects by binding to intracellular glucocorticoid receptors, which then translocate to the nucleus and modulate gene expression. This leads to a reduction in the production of pro-inflammatory cytokines, chemokines, and other inflammatory mediators. Additionally, dexamethasone stabilizes lysosomal membranes, reduces capillary permeability, and inhibits the migration and proliferation of immune cells. These actions collectively result in significant anti-inflammatory and immunosuppressive effects.
Application
Dexamethasone injection is used to treat a wide range of conditions, including:
- Inflammatory Conditions: Such as rheumatoid arthritis, dermatitis, and ulcerative colitis.
- Allergic Reactions: Including severe allergic reactions and asthma exacerbations.
- Autoimmune Diseases: Like lupus and multiple sclerosis.
- Respiratory Disorders: Such as croup in children and bronchial asthma.
- Neurological Conditions: Including cerebral edema and acute exacerbations of multiple sclerosis.
- Ophthalmic Conditions: Such as uveitis and post-surgical inflammation.
- Hematological Malignancies: Used in combination with other chemotherapeutic agents.
- COVID-19: For severe cases requiring supplemental oxygen or mechanical ventilation.
Dosage Instructions
The dosage of dexamethasone injection varies depending on the condition being treated:
- Inflammation: Initial doses range from 0.75 mg to 9 mg per day, divided into 2 to 4 doses.
- Acute Multiple Sclerosis Exacerbations: 30 mg orally daily for 7 days, followed by 4 to 12 mg daily for 1 month.
- Cerebral Edema: 10 mg intravenously, followed by 4 mg intramuscularly every 6 hours.
- COVID-19: 6 mg once daily for up to 10 days.
- Allergic Reactions: A typical regimen starts with 4 to 8 mg intramuscularly on the first day, followed by oral doses over 6 days.
- Important Note: Dexamethasone should be administered under medical supervision. The dosage and duration of treatment should be adjusted based on the patient's response and the severity of the condition.
Storage
Dexamethasone injection should be stored in a cool, dry place, protected from light and moisture. The storage temperature should be between 15°C and 30°C (59°F to 86°F) unless otherwise specified. Keep the container tightly closed to maintain stability and efficacy.
